Winter- Boys Basketball, Cheerleading/Step, Girls Basketball, Wrestling

Spring- Baseball, Boys Lacrosse, Girls Lacrosse, Softball, Track

Registration for Fall Sports will Open on 5/15/24

Note: Baseball and softball are cut sports.

Students can only register for one sport. 

 

****Please read through all of the steps BEFORE proceeding with the registration process or click here to watch a video.***

The registration process can take 2 to 3 weeks from the time you hand in the paperwork until the time you are cleared.

Step 1: Health Forms

If you HAVE a valid physical recorded in the FMS health office dated 3/4/23 or after, complete these forms:

 

If you DO NOT have a current physical on file dated 3/1/23 or after, complete these forms:

Action Plans: If your son or daughter has ever been diagnosed with any of the following medical conditions, please have the examing physician complete the forms found here and make sure you attach to your physical paperwork.

Note:

  • STUDENTS MUST PHYSICALLY HAND SPORTS PAPERWORK TO ONE OF OUR NURSES DURING SCHOOL HOURS ONLY or PARENTS/GUARDIANS CAN DROP OFF FORMS DURING THE SPECIFED HOURS LISTED ABOVE. STUDENTS ARE PERMITTED TO TURN IN FORMS TO THE NURSES DURING HOMEROOM.  IT CANNOT BE DROPPED OFF IN THE MAIN OFFICE, FAXED OR EMAILED.

Step 2: Online Registration 

You must complete the online registration for every sport each season. Student ID number is required for online registration. 

The athletic registration needs to be completed by a parent/guardian AND student together.

Returning User:  The electronic registration keeps your information recorded for later seasons.   Please log back into your account as a "returning user-login here" and click on the Register tab, click on your child's name and then select the sport and click submit.  Once your registration is complete you will receive an email confirmation.

 

New User:  Please log in as a "new user-create an account" and complete the form.  

Please make sure at the end of the process you create a username and password this is the final step in the registration.  

If the final step is missed your registration will not be processed.
